Activity Liability Waiver and Release Form
I (hereinafter referred to as the "Participant") am participating in the pickleball activities and related training entirely of my own free will. I hereby confirm and agree to the following terms:
1. I am in good physical health and have no known medical conditions or illnesses that would prevent me from safely participating in this sports activity. I fully assume all risks regarding my own health and safety.
2. I clearly understand that pickleball and related physical activities involve inherent risks (including but not limited to strains, sprains, collisions, or other accidental injuries). Should any physical injury, illness, property loss, or other damages occur during the activity, I will not hold the Organizer, the venue management, or any of their staff legally liable under any circumstances.
3. If any damage or injury is caused to me by other participants or their guests present at the venue during the activity, I agree to resolve the matter independently and will not file any claims for compensation or reimbursement against the Organizer.
4. I agree to strictly abide by all relevant rules of the activity, venue regulations, and safety guidelines provided by the coaches or staff. I will also show utmost respect to other participants and the Organizer's personnel.
5. The Organizer reserves the right to modify venue usage, adjust activity schedules, determine participant skill levels, limit capacity, and manage other related arrangements at its sole discretion. The Participant shall raise no objection to such changes.
