Waiver of Liability and Assumption of Risk

Assumption of Risk: I understand that participation in sports activities, including but not limited to Pickleball and other court sports, involves inherent risks of physical injury. I voluntarily assume all risks associated with participation, including but not limited to falls, collisions, equipment-related injuries, and any other injuries that may occur.

Health Declaration: I confirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in sports activities.

Release of Liability: I release and hold no liability to the organizers and agents from any and all claims, damages, losses, or injuries arising from my participation in the sport activities;

Rules: I agree to follow all posted rules, regulations, and instructions from the organizer. I understand that failure to comply may result in removal from the premises without refund;

Personal Property: I understand that the organizer is not responsible for any lost, stolen, or damaged personal property;

Photo/Video Consent: I consent to being photographed or recorded during the events for promotional purposes, unless I notify the organizer;

I have read this waiver carefully and understand its contents. I voluntarily acknowledged with full knowledge of its significance.
